2. Complete the Form:

   Print out the downloaded form and fill in all the required details accurately. Ensure you sign and date the form.

   Checklist Documents:
   Please ensure you include the following documents as applicable:

   • Identification Card
   • Death Certificate (if applicable)
   • MF008 Form (if applicable)
3. Email the Completed Form:

   Once the form is fully completed and signed, and all necessary documents are attached, please send it to our official email address:
